That looks like that 2.99 a ft. stuff from Home Depot. Did you place any magnets on that board that's 3 rows out from the door?

Who tracked that mud in? Smiler My guess would be moisture intrusion from in or around that patio door.

I've heard tales about installers who would deliberately skip some rows in nailing. In this case it sounds like their nailer/stapler ran out of fasteners and kept on firing. Mine will do that.

That floor has no integrity and i would veto a simple sand / finish remedy.

I say "sand the floor".
If it works, fine.
If it does not work;;; not so fine.

This gives the Inspector a second inspection, no matter what happens. Smiler

Gotta love those moisture intrusion jobs. I wouldn't sand it...it doesn't even look like a solid product.
